Compass Healthcare Consulting & Placement is conducting a search for a Hospice Medical Social Worker position visiting patients and their family members who are at an inpatient unit, nursing home or home care setting in Brooklyn, NY. Additional openings for making visits in the Bronx, NY OR Queens, NY as well. Qualified candidates will have prior Hospice or Home Care Social Worker or bereavement counseling experience as well as the ability to travel to patient locations. Opening available within Brooklyn, the Bronx, or Queens, NY. Driver's license and access to vehicle, required.

The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and their family members at an inpatient unit, nursing home or home care setting. Position requires travel. Visit areas available in Brooklyn, NY, Bronx, NY or Queens, NY.

Qualifications:

• Minimum one (1) year bereavement counseling experience is preferred.
• At least one year of Hospice or Home Care experience preferred.
• New York State driver’s license required.
• Excellent interpersonal, and communication skills.
• Exhibits an ability and attitude relative to the spirit of caring as required by specific job duties and functions.

Education:

• MSW from an accredited school of social work and one year related experience.
• LMSW or LCSW required.

Competitive Salary from $68,000 to $94,955, depending on licensure and experience. Some positions offer sign-on bonus of up to $15,000! Excellent Benefits!
